If you would like a vision and/or hearing screening at your work, church, or event, please fill out the form below (note: we will only provide equipment for groups where the expected turnout is 15 or more people).
Please note that this is a health education screening to determine if someone needs to go to a doctor and is not a vision or hearing exam.
